Student ID Photo Submission and Parking Permit Information Form

(Complete this only if you do not already have a Mercy College Student ID)

• A device with a camera is required.
  • Computer users generally will need to select “Use Rear Camera” before taking your photo.
  • iPhone users must install the Microsoft PowerApps app for camera access.
  • Android user can either install the app or select “Continue in browser”.
• Log in using your Mercy College Microsoft 365 account (first.last@my.mchs.edu)
• Follow the on-screen instructions to take/upload your photo and enter your vehicle details.  Make sure you check the boxes and select submit to see the final confirmation screen.
• You will be emailed when your badge is ready for pickup usually within two to five business days.
