MLS Program Application
MEDICAL LABORATORY SCIENCE
PROGRAM APPLICATION
The MLS Program Application is to be used by all students applying to the Medical Laboratory Science program. This includes current Mercy College students, students from affiliated and non-affiliated institutions, and those who already hold appropriate Bachelors or higher degrees.
Before you begin this application, please have the following information available in order to be able to complete all sections:
- Applicable health care licensures & certificates
- Employment history (last 3 positions)
- Current Course Schedule*
- Planned Course Schedule
- Academic Record (Biology/Microbiology/Zoology courses; Chemistry/Biochemistry courses; Mathematics/Physics/Statistics courses)
- Name, title and contact info for three (3) references
- Essay (500 word max) describing your career goals and why you are choosing a career in Medical Laboratory Science.
*This step is only for students who are NOT currently attending Mercy College.
START APPLICATION
The application consists of the five sections identified below and will take approximately 30-60 minutes to complete. If you are unable to complete the entire application at one time, each section of the application will be saved separately when you click “submit”. Incomplete sections will not be saved. Once you click “submit”, changes to the section cannot be made.

REFERENCES
Once you have completed your application, you will receieve an email to be forwarded to your selected references. If you do not receive this email, please contact your references and direct them to fill out and submit the MLS Applicant Reference Form before October 15.
