Student Health Verification
Following admission to the College, all medical record forms must be completed and on file with the College Student Health Nurse before clinical educational programs begin.
Complete and return the following College-supplied forms prior to or at your designated New Student College Orientation/Registration session:
- MCHS Medical Examination/Physical form signed by a physician. Medical Examination/Physical form must have been completed within the last 12 months.
- Clinical Standards form signed by BOTH applicant and physician. Clinical Standards must have been completed within the last 12 months.
- Immunizations records. Document immunizations on the Medical Examination/Physical form or verify immunity to the following diseases: Measles, Mumps, Rubella, Varicella, Tetanus and Hep B.
- Two-step tuberculin skin tests. Document the complete two-step tuberculin skin tests screening on the Medical Examination/Physical form.
- Medical History/Health Update form signed and dated by the student.

Students need to retain copies of all medical record forms submitted for admission to the College. Once submitted all health documents become the property of the College and are not released or returned to students. Health documents are maintained in the Student Health Nurse's office and are not part of a student's academic record.
